Welcome to Step 1 of the Pre-participation evaluation. This portion of the PPE is meant to provide you very important, up to date information, regarding medical eligiblity.

We recommend including your parents when reading this portion of the evaluation as important information regarding medical services, insurance information, and NCAA Banned Substances have been included for your review.

It is the responsibility of the student-athlete to be aware of the following information and how it applies to medical benefits and eligibility during the course of their experience at Stanford University.

• USADA Banned / Prohibited Substance List
• New IMPORTANT Information for Student-Athletes regarding perscription medication for Attention Deficit Disorder (ADD) and/or Attention Deficit Hyperactivity Disorder (ADHD)
