ALL Student-athletes are required by the NCAA to undergo an annual physical examination prior to the start of each competitive athletic season. Unless otherwise instructed by your team Athletic Trainer or Team Physician, ALL Preparticipation Physical Examinations will be provided by Stanford Sports Medicine.

Physical examinations can only occur when the Online ePPE portion of the medical evaluation has been completed and submitted by the student-athlete. Preparticipation Physical Examinations performed by Stanford Sports Medicine include the following components

• Check-In - Review of Medical and Insurance Information
• Vital Signs - Height, Weight, Heart Rate, Blood Pressure
• Vision Screening
• General Medical Screening
• Orthopaedic Screening
• Cardiac Screening - Electrocardiogram (ECG), Ecchocardiogram - FRESHMAN ONLY
• Check-Out

Online ePPE forms completed by returning student-atheltes prior to August 1, 2009 will be reviewed by Stanford Sports Medicine personnel. Student-athletes that are determined to be a “healthy returning student athlete” will not have to be present for the physical examination portion of the pre-participation evaluation. All returning student athletes part of Stanford Football will be required to be present at their physical examination.

ALL incoming and freshman student athletes are required to be present at their program's scheduled physical examination.

It is very important to arrive at your physical examination on time wearing proper attire. Please view the Physical Exam schedule below to find your particular date and time. Attire guidelines will be enforced and have been delineated below for your convenience. Student atletes arriving to their physical examination wearing innapropriate attire will be dismissed and asked to change into proper attire.

All Preparticipation Physical Examinations will occur at the Lacob Family Sports Medicine Center which is located on the bottom floor of the Arrillaga Family Sport and Recreation Center at the corner of Galves St. and Campus Drive (immediately across Campus Drive from Cobb Track and Angel Field complex).
